Essential Functions/Responsibilities
- Maintain an ongoing caseload of a minimum of 40 clients for intensive case management including “warm handoff” linkages to: housing; legal services; help securing documents like driver’s license, identification, services; court mandated classes; tattoo removal; psych/medication monitoring; job search services; skill building such as resume writing; enrollment in education and/or vocational programs; and enrollment in college, certification programs, and other long-term/wait list programs for adults who have been recently released from incarceration and re-entering the community.
- Participate in interdisciplinary teams inclusive of clinical and non-clinical, program driven staff.
- Develop case management plans with clients that include short/long-term re-entry goals and objectives.
- Work closely with Clinicians, Peer Navigators and county partners to ensure seamless transitions and that client treatment plans are met.
- Collaborate effectively with other community organizations, county services, school districts, and wrap around services to establish a continuity of care for “warm handoff” linkages for clients.
- Submit requests to the Clinical Coordinator for Flex Funds and Support Services that are appropriate to treatment plan goals; document and track client usage of such items in their case files.
- Maintain case files in database in a ready state for audits according to quality assurance and compliance standards.
- Ensure HIPAA, ethical code of conduct, and professional boundaries are maintained with staff and clients.
- Compile and submit reports, demographics, and other required evaluation data to the Data Analyst and Clinical Coordinator in a timely manner.
- Commit to the agency’s Mission, Vision, and Values; represent actions that define the overall philosophy and objectives of the agency.
- Work a flexible schedule outside of business hours, including weekends and evening hours, as the need arises or to support the program and leadership.

Education and Experience
- Bachelor’s degree from an accredited university in the field of Human Services, Social Work, Counseling, or related field or equivalent work experience.
- Minimum of two years of experience working within a mental health program providing case management, rehabilitation, crisis response, and trauma-informed services is required.
- Must complete a Live Scan Background Check.
- Experience working with gang-involved, incarcerated, recovery, and/or re-entry populations and their families is preferred.
- Experience in working within county contract reporting requirements is preferred.
- Intermediate PC skills required; experience in working with Electronic Medical Record systems preferred.
- Bilingual in Spanish or other threshold languages is preferred.

- Knowledge of the steps towards Recovery and Wellness utilizing a Trauma Informed Approach.
- Understanding of re-entry, formerly incarcerated, immigrant, and refugee cultural issues and dynamics.

- Background Check Requirement: The successful candidate for this position will be required to pass or clear two background checks. The first background check will be conducted by Project Kinship as per our policies and procedures. The second background check will be conducted by Funders through our contracted vendors, after the candidate is hired with Project Kinship, in accordance with our contractual agreements. Failure to pass either of these background checks may result in the withdrawal of the job offer or termination of employment.
- Tuberculosis (TB) Clearance: The successful candidate for this position will be required to provide documentation of a clear TB (tuberculosis) test or shot, in compliance with Project Kinship’s policy.
