Key Responsibilities: 

• Deliver personalized nursing care based on individual care plans 
• Monitor and document residents' health status, ensuring timely interventions 
• Work collaboratively with a multidisciplinary team to develop and implement care plans
• Assist residents with daily living activities and personal care as needed 
• Maintain accurate records and ensure compliance with health and safety regulations 

What We Are Looking For: 

• Licensed Practical Nurse (LPN) with a valid, current license 
• Experience in mental health care (preferred by not required) 
• Strong communication and teamwork skills 
• A compassionate, patient-centered approach to care
• A positive attitude and a commitment to improving the residents' quality of life
