Looking for LPNs and CMAs who are proactive, resourceful, and passionate about patient care! This is a fully remote role in Alabama, Arkansas, Florida, Georgia, Mississippi, Tennessee, Texas, Kentucky, Oklahoma, or Virginia. The Chronic Care Management position is full-time, Monday to Friday during the day.

In this role, you will be providing one-on-one attention to patients with chronic diagnoses through telephone calls. You will collect health information, provide education, and encourage treatment compliance. LPNs can earn up to $24 per hour and CMAs can earn up to $17 per hour. We provide all the tools, you just need the internet and a quiet and private office space at home.

Requirements:

• Must be an LPN or CMA.
• High-speed internet required (no satellite internet or hotspots).
• Proficiency in Microsoft Outlook and Excel.
• Experience with EMR documentation.
• Self-motivated, able to work remotely, and enjoy helping others.

Responsibilities:

• Conduct initial risk assessments to create patient-specific Care Plans.
• Collaborate with patients to plan and monitor their care.
• Encourage compliance with office visits and medication.
• Act as a patient advocate and provide education on chronic conditions.
