PRIMARY DUTIES AND RESPONSIBILITIES:
1. The provision of hands-on personal care.
2. The performance of simple procedures as an extension of therapy or nursing services.
3. Assistance in ambulation or exercises according to the plan of care.
4. Assistance in administering medications ordinarily self-administered.
5. Recording pertinent information pertaining to the clients care and strict adherence to the HHA care plan that is on the clients chart.
6. Providing hands-on personal care including: Baths, oral hygiene, shampoos, assisting with/changing bed linens, assisting patients with dressing/undressing, skin care to prevent breakdown, assisting patients with toileting activities, keeping patients living area clean and orderly, as appropriate.
7. Planning and preparing light meals/snacks according to specific dietary requirements of the client, and if necessary, feeding the client.
8. Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
9. Performing simple procedures within the HHAs scope of practice as an extension of nursing services as ordered (with appropriate completed/demonstrated skills competency).
10. Meeting the safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
11. Encourages and maintains safety standards in the workplace and home.
12. Participates in an on-call rotation as assigned for holiday and weekend coverage.
13. Taking clients vital signs and recording input and output as specified on the HHA plan of care (with appropriate completed/demonstrated skills competency).
14. Observing and reporting on the patients condition and changes in patient's condition to the Care Manager/clinician, and /or Clinical Services Supervisor.
15. Adhering to the agencys documentation and care procedures and standards of personal and professional conduct.
16. Consistently demonstrates timely communication with agency staff, supervisors, clients, client family and representatives, etc. Promptly returns phone calls.
17. Participating in in service education programs so as to meet regulatory guidelines.
18. Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service.
19. Consistently represents all company entities to all internal and external customers with integrity and professionalism.
22. Performs other duties as assigned by management.

QUALIFICATIONS:

1. Meets the training requirements and successfully completes:

A. A training and competency evaluation program that meets 484.80 (b) and (c); or

B. A competency evaluation that meets the requirements of 484.80 (c); or

C. A nurse aide training and competency evaluation approved by the state as meeting the requirements of 483.151 through 483.154 and is currently in good standing on the state nurse aide registry; or

D. The requirements of a state licensure program that meets the provisions of 484.80 (b) and (c).

2. CNA Certification required (*for Certified Nursing Assistant position only).

2. At least 18 years of age.

3. High School diploma or equivalent.

4. Ability to read, write, and follow written instructions, and document care given.

5. Self-directing with the ability to work with little direct supervision.

6. Empathy for the needs of the ill, injured, frail and the impaired.

7. Related work experience preferred.

8. Must be physically able to perform the duties of the position.

9. Must exhibit mature responsible behavior, and understand the need for patient confidentiality.

10. Must be able to follow direction and accurately report to the Care Manager/clinician any changes in client's condition.

11. Possess and maintains current CPR certification.

12. Demonstrates tact, patience and good personal hygiene.

13. Licensed driver with automobile that is insured in accordance with Organization requirements and is in good working order.

JOB LIMITATIONS:

The HHA will not function in any manner viewed as the practice of nursing according to the states Nurse Practice Act. Specifically, the HHA will not administer medications, take physicians orders or perform procedures requiring the training, knowledge and skill of a nurse, such as sterile techniques.
