Job Description
Are you an experienced Licensed Master Social Worker who is passionate about hospice and palliative care? The Outpatient Hospice deparment is seeking an experience LMSW who will serve as subject matter expert for Hospice program. In this role you will assist leadership with clinical, quality, educational and strategic initiatives. Supports, patient safety and clinical practice standardization. Assists with inter-professional collaboration and communication. Supports optimal utilization of the Epic system both hyperspace and remote client, and other technologies to improve documentation and care planning. Helps implement specific clinical practice guidelines, serves as preceptor, mentor and advisor to staff, providers and leadership. Supports statewide initiatives as appropriate.

ResponsibilitiesIntegrates Epic tools and systems in daily practice to document and communicate within social worker team
Provides coaching in the moment at the point of care for social workers
Represents social workers as subject matter expert for the implementation of new technology and equipment.
Fosters positive attitudes and demonstrates leadership as a team player.
Models interprofessional collaboration
Follows and models clinical practice supporting patient safety initiatives.
Apply evidence-based practice knowledge/critical thinking ability and expertise in the social work setting with patients.
Effective in mentoring social workers to follow the changes in documentation and practice.
Models best practices and follows department practice, policy, procedure and/or guidelines for patient care.
Supports deployment of educational programs to support implementation of upgrades relating to Epic, policy, or practice changes
Effective in mentoring staff to follow the changes in documentation and practice
Collaboration with nursing managers to review quality and performance driven outcomes, to ensure that the use of current evidence-based outcomes, to ensure that the use of current based practice is implemented
Proactively identifies performance gaps and collaborates with the nursing managers first, then teams and individuals to share performance data and plan for improvement
Provides feedback to peers regarding documentation and evidence-based practice standards to achieve quality and performance goals in collaboration with department leadership.
Completes supervisory visits with all social workers twice a year with a summary of visit discussion with department leadership.
Attend monthly QAPI meetings
Provides supervision meetings with BSW and those working on higher level certification
Weekly 1:1 meeting with nursing manager for planning and providing updates on projects
Develop the agenda and oversee the social worker meeting monthly for case reviews
Attend monthly meetings to support department leadership with agenda development and note taking during meetings.

Qualifications- Master Degree in Social Work LMSW
- Licensed Master Social Worker in the State of New Mexico
