JOB SUMMARY
As a member of the interdisciplinary team, assesses the needs, interests and capabilities of participants and develops individualized therapeutic recreation plans. In addition, plans, organizes, directs and participates in a comprehensive therapeutic recreation program tailored to the general and individualized needs and limitations of frail elderly program participants. Provides leadership, supervision and support to center staff.
POSITIONS SUPERVISED
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides high quality care and serves as a member of the PACE interdisciplinary team (IDT)
- Performs in-person initial assessments for enrollment of potential PACE participants to obtain an understanding of functional abilities and needs, goals, history, capabilities, and interests.
- Assesses the need for socialization, recreation, emotional support and plans activities and other interventions to meet identified needs.
- Plans and implements group and individual activities to improve or maintain the participants cognitive, physical, social, psychosocial and artistic needs.
- Plans and implements activities for dementia-specific population. Oversees daily flow of dementia activities area. Activities may focus on small groups, one-on-one programming and sensory stimulation.
- Organizes community outings and special events at the PACE Center.
- Develops and posts a monthly calendar of activities.
- Evaluates plans to promote effective programs according to client abilities and preferences within the PACE Centers daily flow of events.
- Provides recreational educational and training to support Recreation and Activities Assistants and other Center staff members as appropriate.
- Provides on-going support and training to all staff involved in dementia-specific programming related to the dementia disease process.
- Documents evaluation, assessment, plan and treatment rendered accurately and in timely according to policy.
- Measures effectiveness of treatment, documents and revises plan accordingly.
- Directs, supervises, and performs initial and annual competencies, and evaluates the performance of the Activities Assistant and other staff as indicated.
- Provides care and services in the center, clinic, contracted facilities, and participants homes according to each participants plan of care.
- Assists participant to function at most independent level possible.
- Performs home visits as needed to assess the needs of participant to remain as active as possible in their home setting and continue to pursue leisure interests.
- Provides leisure education to participants and caregivers for activity modification, use of adaptive equipment, activity skill development and recommended supplies for in-home use.
- Maintains general order in the activities area, including daily clean-up, routine supply inventory ordering, petty cash and overall general budgeting.
- Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
- Develops and maintains positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
- Participates in all interdisciplinary team meetings
- Reviews and revises goals and approaches to participants care in coordination with interdisciplinary team.
- Assists the interdisciplinary team members in understanding the significant rehabilitation needs of the participant and families
- Collaborates with the interdisciplinary team to develop a comprehensive care plan for each participant.
- Maintains regular attendance at and participates in daily Interdisciplinary Team meetings, communicates participant changes and collaborate with team members in care planning decisions and coordination for 24-hour care delivery.
- Acts as liaison as needed with participant, caregivers, and community agencies regarding orientation to and ongoing relations with Interdisciplinary Team, day center, and other PACE staff.
- Performs the duties of other IDT members based on professional licensing, competencies, and experience as needed.
- Actively participates in utilization review meetings and quality improvement projects and meetings.
- Attends and actively participates in a variety of organizational meetings related to participant care or daily operations, in-services and community agency meetings.
- Completes all documentation of clinical service in participants medical records, including initial assessments; reassessments; change of status; and other significant events according to PACE documentation requirements.
- Follows all Policies and Procedures and OSHA safety guidelines.
- Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families.
- Practices standard precautions.
- Maintains safe working environment, following safety policies and procedures.
- Participates in continuing education classes and any required staff and training meetings.
- Maintains professional affiliations, required certifications and continuing education requirements.
- Other duties as assigned.
EXPERIENCE AND EDUCATION
- Recreation Therapist / Bachelor degree or Certified Activities Coordinator (ADC) by the National Certification Council for Activity Professionals
- Current drivers license and proof of auto insurance.
- Experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.
- Minimum of 1 year working with an elderly population required.
