FUNCTION: The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

QUALIFICATIONS: The requirements listed below are representative of the knowledge, skill and/or ability required.

Minimum Qualifications:
Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services.
Ability to remain calm in emergency or crisis situations.
Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
At least one year of experience as a physical therapy aide.
Current license from the state Board of Physical Therapy Examiners.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each key function satisfactorily.
Responsibilities: - Provides direct patient care, evaluates needs, consults with specialists, and adjusts care plans. - Designs and implements patient care based on assessments and involves significant others. - Conducts therapy within defined standards and directs support staff. - Communicates effectively to achieve positive patient outcomes.
