Manager of Social Care
Position Purpose:
The Manager of Social Care provides program direction and oversight including planning, development and implementation of program services, budgetary and resource planning and management. Establishes and develops collaborative relationships within the Health System and community that promote the Mission of Mount Carmel/Trinity Health. In collaboration with Mission leadership, develops and implements a strategic plan for identifying and addressing patient needs, disparities and barriers to care encountered by high-risk and vulnerable populations. Provides direct oversight of HUB Case Management and Community Health Workers to ensure effective, evidence-based care coordination and navigation for persons in high-risk categories. Provides support and direction to Community Health & Well-Being colleagues, Mount Carmel Health System Colleagues, and community partners to identify and implement opportunities to bridge gaps and provide innovative and culturally competent solutions for continuity of care.
- Education: Bachelors Degree, preferably in Human Services, Social Work or related field
- Licensure / Certification: Active CPR certification
- Experience: Minimum of 7 years community/social services related work; minimum 3 years manager or supervisory/leadership experience
- Experience working with vulnerable and culturally diverse populations required
- Effective Communication Skills both written and verbal
- Demonstrated ability to work in and facilitate a team environment
- Demonstrated ability to lead and influence staff, colleagues and community partners to achieve positive outcomes
- Awareness of issues and resources related to vulnerable populations such as homeless, undocumented, uninsured or impoverished
- Demonstrates the ability to manage multiple programs and projects effectively
- Ability to identify potential disparities in healthcare and actively lead strategic planning and implementation to address need according to Mission of Mount Carmel/Trinity Health
