The Admissions Outreach Coordinator will work very closely with the Care Transition Manager and be responsible for any and all administrative duties that are necessary to execute all admissions and keep the 120 bed facility within compliance. This role will also be directly responsible for helping to generate long term care stay referrals and admissions.

Essential Responsibilities:

  • Be responsible for correcting daily reporting of the Census, including insurance updates
  • Be responsible for daily upkeep and accurate accounts of the referral Log
  • Attend morning admissions meetings and report on upcoming admissions, and attend monthly collections meetings
  • Meet long term care census goals via the execution of outreach tactics specifically developed to drive in long term care admissions, including, but not limited to: visiting/contacting hospitals and area agencies to generate new long term care referrals for the community
  • Fully execute the completion of WEL's long term care application and assist families in expediting required documentation for all long term care referrals that come into the community; work collaboratively with finance department to obtain timely approvals/any supporting documentation requested
  • Provide hospital visitation duties to departments of case management to support Care Transition Manager as needed
  • Provide backup coverage to other WEL SNF's to coordinate incoming admissions, as needed.
  • Perform other duties as required.

Qualifications

Related Experience:

  • 2-4 years administrative admissions experience in a hospital or skilled nursing environment; Strong knowledge of Medicare, Medicaid and other insurances

Education or Degree Required:

  • High School Diploma or equivalent; Some College preferred

Licenses, Certifications, Registrations:

  • Valid Driver's License; Current Car insurance
