Home care Coordinator

Job Description:

1. Coordinate and provide home care services to clients in the Boynton Beach area
2. Conduct psychosocial assessments and provide supportive counseling to clients and their families
3. Work with community resources to ensure clients receive the best possible care
4. Collaborate with other social service professionals to develop care plans and interventions
5. Provide crisis intervention and mental health counseling as needed
