• GD Foothill - 8880 E. Foothill Blvd, Ste 5, Rancho Cucamonga, CA 91730

Position Qualifications:

• Minimum 1 year of General Dentistry experience
• Accredited Dental School Graduate
• Current applicable State Dental License (or the ability to acquire one by time of employment)
• Other licenses/certifications such as CPR, DEA, NPI
