NURSE ( RN / LPN ), MENTAL HEALTH, ACT TEAM

The Assertive Community Treatment team (ACT) in Nelson County is dedicated to providing comprehensive mental health care and support to severely mentally ill adults. We are seeking a RN or LPN to join our team. In this role you will collaborate closely with a multidisciplinary team to provide holistic and person-centered care and support ensuring that consumers achieve their highest level of functioning and well-being in the community. Major duties include the following: 

• Coordinate care with community healthcare providers and our team psychiatric provider

• Participate in team meetings, case conferences, and ongoing professional development

• Administer injections and lab orders

• Facilitate health promotion and illness prevention activities, including lifestyle management and symptom management.

• Engage with consumers and their families to support treatment adherence and improve health outcomes.

• Maintain accurate and up-to-date consumer records and documentation.

• Provide crisis intervention and support to consumers experiencing acute psychiatric episodes.

Position requires strong organizational and time management skills, excellent writing skills, flexibility, and self-motivation. 

Valid VA driver's license and acceptable driving record is required. 

The incumbent in this position must be a registered nurse (RN) with one year of experience in the provision of mental health services to adults with serious mental illness or a licensed practical nurse (LPN) with three years experience in the provision of mental health services to adults with serious mental illness. 

Salary and hourly rate is negotiable based on experience. Minimum Hourly Rate $24.22.
