Venesco, LLC has an opening for a nights Clinical Pharmacist to provide services at Eglin Air Force Base, FL. Eglin AFB is situated in the beautiful coast region near Fort Walton Beach. This location is near beautiful beaches and golf courses. This schedule affords you the opportunity to spend your days relaxing with every other week off!
Competitive pay & benefits offered including:
Hours of Performance: Normal duty hours start between 6:00 and 8:00 pm and ends between 6:00 am and 10:00 am, 10 to 12 hours per shift up to 84 hours in a two-week period. The typical schedule will be 7 days on 7 days off over a 2-week period. Breaks will vary depending on work requirements and workload.
Duties:
1. Assess patient's response to drug therapy and planning drug therapy based on physician-established diagnoses.
2. Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes.
3. Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff.
4. Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics ( e.g., aminoglycosides, phenytoin, antithrombotic ).
5. Administer prescription or non-prescription drugs according to established agreements or MTF protocols.
6. Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting.
7. Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly.
8. Compound medications as required.
9. Consult with other healthcare providers ( e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options.
10. Counsel patients regarding medications, including patients who are discharged.
11. Monitor and manage medication dispensing units (i.e., Pyxis) for appropriate removal, replenishment, and clinical review.
12. Provide guidance in dispensing medications, making purchasing decision, verifying calculations, and preparing IV medication, and overseeing drug administration.
13. Respond to, and participate in, any facility emergency codes as required by the facility.
14. Perform a full range of pharmacist procedures.
Requirements
1. Degree/Education: Successful completion and graduation with Doctor of Pharmacy (Pharm.D.) from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
2. Certifications: In addition to Basic Life Support, must possess American Heart Association or the American Red Cross Advanced Life Support (Adult) certification.
3. Licensure/Registration: Shall have and maintain a current, full, active, and unrestricted license to practice as a pharmacist in any one of the 50 states, the District of Columbia, the Commonwealth of Puerto Rico, or the U.S. Virgin Islands.
4. Experience: The pharmacist shall have at least 12 months of inpatient pharmacy experience within the previous 36 months practicing as a pharmacist as well as at least one of the following 3 options:
· Option 1: Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy.
· Option 2: Board Certification in one or more of the pharmacy specialties recognized by the Board of Pharmaceutical Specialties.
· Option 3: Documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
