Licensed Practical Nurse Job Description
The Licensed Practical Nurse must be able to perform the following essential functions as demonstrated by the ability to:
- Collaborate with other departments and professionals to streamline operations
- Willingness to keep abreast of changing standards in nursing administration
- Respond to any nursing-related issues in a timely manner
- Follow online record-keeping processes and ensure all necessary documents are accurate and up-to-date
- Establish good public relations with hospital discharge coordinators and social services
- Direct implementation of “Patient Bill of Rights”
- Evaluate care as related to individualized client / patient needs, family involvement and the physicians plan of care for the client / patient.
- Maintain patients’ electronic data and electronic medical records
- Tracks and documents all client / patient interactions, analyzes the associated data and makes recommendations to management to improve the client / patient experience
- Great communication and people skills
- Outstanding organizational skills
- Consistently demonstrate responsibility, accountability and dependability
- Apply the 48 hours rule, at all times, in the best interest of the agency

Requirements:
- Degree in Nursing or equivalent experience
- Homecare experience preferred
- In-depth knowledge of legal policies and regulations in the healthcare industry
- Certified nursing and CPR qualifications
- Aptitude for problem solving
- Experience in following and maintaining workplace privacy
- Excellent leadership and interpersonal skills
- Outstanding written and verbal communication skills
- Ability to work independently or with the team
