Licensed Practical Nurse Job Description

The Licensed Practical Nurse must be able to perform the following essential functions as demonstrated by the ability to:

• Collaborate with other departments and professionals to streamline operations
• Willingness to keep abreast of changing standards in nursing administration
• Respond to any nursing-related issues in a timely manner
• Follow online record-keeping processes and ensure all necessary documents are accurate and up-to-date
• Establish good public relations with hospital discharge coordinators and social services
• Direct implementation of “Patient Bill of Rights”
• Evaluate care as related to individualized client / patient needs, family involvement and the physicians plan of care for the client / patient.
• Maintain patients’ electronic data and electronic medical records
• Tracks and documents all client / patient interactions, analyzes the associated data and makes recommendations to management to improve the client / patient experience
• Great communication and people skills
• Outstanding organizational skills
• Consistently demonstrate responsibility, accountability and dependability
• Apply the 48 hours rule, at all times, in the best interest of the agency

Requirements:

• Degree in Nursing or equivalent experience
• Homecare experience preferred
• In-depth knowledge of legal policies and regulations in the healthcare industry
• Certified nursing and CPR qualifications
• Aptitude for problem solving
• Experience in following and maintaining workplace privacy
• Excellent leadership and interpersonal skills
• Outstanding written and verbal communication skills
• Ability to work independently or with the team
